This form supports multiple file uploads for jpg, png, and pdf. There is no limit to number of uploads, but total cannot exceed 100MB. To upload more than one file, hold the command key (Mac) or Ctrl (PC) while selecting the files. To upload from your mobile device, simply select the files you wish to upload.
